Advanced Practice Provider, Nurse Practitioner, Or Physician Assistant

Summary

Join Movn Health, a leading virtual cardiac rehab program, as a contracted Advance Practice Provider. You will oversee patients, conducting telehealth visits to facilitate referrals for RPM and CCM programs. This role involves collaborating with the clinical team, communicating with referring physicians, and engaging with healthcare leaders to promote Movn's innovative approach to cardiac care. The ideal candidate possesses strong communication and people skills, along with a background in clinical cardiology or primary care. The position requires approximately 5-10 hours of work per month.

Requirements

• Nurse Practitioner or Physician Assistant
• Board certified with Master’s degree or Graduate of an accredited Physician Assistant Program
• Current Nurse Practitioner or Physician Assistant License in Massachusetts and New York
• Experience in cardiac care

Responsibilities

• Provide Evaluation and Management telehealth visits to gain referral for RPM and CCM programs
• Serve as physician champion/liaison for our remote cardiovascular risk prevention program for the region
• Work with our clinical team to ensure that all patients are being appropriately treated and triaged as needed
• Communicate and Interface with referring physicians about individual patient outcomes and progress toward goals
• Engage leadership at health systems, health plans, department of health, physician practices, Cath labs on the need for alternate delivery models for cardiac rehab and educate them on the benefits of the Movn program

Preferred Qualifications

• Experience in remote patient monitoring and chronic care management
