Requirements
- Registered Nurse degree from an accredited school of nursing is preferred but not required; LPNs will be considered
- Licensed/Certified/eligible to practice in the United States
- Licensure in Michigan (within 3 months of hire) and ability to become licensed in additional states
- Three years of supervisory or leadership experience in a healthcare environment
- Experience with various EHR platforms (Epic, Cerner, Athena, etc.) is required
- Strong organizational skills and ability to manage multiple priorities in a fast-paced environment
- Commitment to quality, safety, and continuous improvement in patient care delivery
Responsibilities
- Manage the day-to-day operations for a team of remote Central Care Navigators by providing direct leadership, support, and guidance
- Assume primary responsibility for hiring, training, and development of Care Navigators in collaboration with the Navigation Service Line Leader
- Become a subject matter expert in all things care navigation for Homeward
- Conduct regular team meetings, one-on-one sessions, and performance evaluations to assess individual and team performance
- Foster a positive and inclusive work environment that promotes teamwork, professional growth, and employee engagement
- Conduct regular quality assurance reviews via direct observation of virtual member visits and management of member feedback
- Conduct regular audits of member records, documentation, and care plans to ensure compliance with regulatory standards and company guidelines
- Identify opportunities for quality improvement and implement corrective actions as needed to enhance the quality and effectiveness of member care services
- Serve as a point of contact for the Central Care Navigators for concerns, grievances, and conflict resolution, providing guidance, mediation, and support as needed in a timely manner
- Collaborate with cross-functional teams to streamline processes, enhance workflows, and drive operational efficiency
- Maintain open lines of communication with internal stakeholders, including other departmental supervisors, clinical staff, and administrative personnel
Preferred Qualifications
- Two years of supervisory experience in a Hybrid Model or Full Remote Model is preferred
- Passion for delivering care in rural America
- Experience working on a small, fast-paced team is a plus
- Experience working with connected devices/loT is a plus
- Experience working in a start-up environment preferred
- Experience working in value based healthcare is a plus
