CCM Manager, Interdisciplinary Pharmacist/Dietitian

Summary

Requirements

• Pharmacist
• At least 3 years of people management leadership experience in a healthcare setting
• Excellent organizational skills and attention to detail required
• Strong interpersonal skills with ability to build relationships with employees
• Data-driven and comfort in using tools to evaluate and optimize the standardization of care across our growing team
• Ability to be agile and balance multiple priorities, while maintaining positive and professional attitude
• Demonstrates professional, appropriate, effective, and tactful communication skills, including written, verbal and nonverbal
• Prompt and regular attendance at assigned work location
• Capability to remain seated in a stationary position for prolonged periods
• Eye-hand coordination and manual dexterity to operate keyboard, computer and other office-related equipment
• No heavy lifting is expected, though occasional exertion of about 20 lbs of force (e.g., lifting a computer / laptop) may be required
• Capability to work with leadership, employees, and members in an appropriate manner
• Occasional overnight business travel

Responsibilities

• Provide overall leadership and operational direction for the interdisciplinary team
• Ensure implementation of strategic and process changes designed to achieve outlined goals/objectives
• Oversee development of department specific goals and objectives, ensuring alignment with Care Management strategies and Included Health's vision, mission, and values
• Monitor team performance and outcomes, ensuring alignment with OKRs and organizational goals
• Oversee day-to-day operations of the team, ensuring efficient workflows and equitable distribution of caseloads
• Ensure that all team members adhere to professional standards, ethical guidelines, and regulatory requirements
• Monitor and evaluate the quality of performance by the team, implementing improvements as needed
• Provide leadership, mentorship, coaching, and professional development opportunities for team members
• Serve as the primary point of contact between the interdisciplinary team and senior leadership
• Prepare and present reports on team performance, patient outcomes, and operational metrics
