Summary
Join Included Health as a Manager, Connected Care Program, and lead a diverse team of healthcare professionals. You will oversee daily operations, resource allocation, and ensure compliance. Drive team performance in delivering high-quality, patient-centered care, addressing patients' medical, psychosocial, nutritional, and mental health needs virtually. This role requires strong leadership, operational management, and quality assurance skills. You will mentor team members and report to the Clinical Director. Requirements
- Pharmacist
- At least 3 years of people management leadership experience in a healthcare setting
- Excellent organizational skills and attention to detail required
- Strong interpersonal skills with ability to build relationships with employees
- Data-driven and comfort in using tools to evaluate and optimize the standardization of care across our growing team
- Ability to be agile and balance multiple priorities, while maintaining positive and professional attitude
- Demonstrates professional, appropriate, effective, and tactful communication skills, including written, verbal and nonverbal
- Prompt and regular attendance at assigned work location
- Capability to remain seated in a stationary position for prolonged periods
- Eye-hand coordination and manual dexterity to operate keyboard, computer and other office-related equipment
- No heavy lifting is expected, though occasional exertion of about 20 lbs of force (e.g., lifting a computer / laptop) may be required
- Capability to work with leadership, employees, and members in an appropriate manner
- Occasional overnight business travel
Responsibilities
- Provide overall leadership and operational direction for the interdisciplinary team
- Ensure implementation of strategic and process changes designed to achieve outlined goals/objectives
- Oversee development of department specific goals and objectives, ensuring alignment with Care Management strategies and Included Health's vision, mission, and values
- Monitor team performance and outcomes, ensuring alignment with OKRs and organizational goals
- Oversee day-to-day operations of the team, ensuring efficient workflows and equitable distribution of caseloads
- Ensure that all team members adhere to professional standards, ethical guidelines, and regulatory requirements
- Monitor and evaluate the quality of performance by the team, implementing improvements as needed
- Provide leadership, mentorship, coaching, and professional development opportunities for team members
- Serve as the primary point of contact between the interdisciplinary team and senior leadership
- Prepare and present reports on team performance, patient outcomes, and operational metrics
