Summary

Join Cerebral's team as a Complex Case Manager (CCM) and provide exceptional long-term mental health care to high-acuity clients. You will be instrumental in crisis stabilization and acute symptom reduction, utilizing your expertise in case management and support services. The ideal candidate possesses strong emotional intelligence, excels in high-stress situations, and proactively approaches client care. This role requires a Bachelor's Degree in Nursing, at least 2 years of psychiatric RN experience, and an active RN Compact License. Requirements

  • Possess a Bachelor's Degree in Nursing
  • Have at least 2 years of psychiatric RN experience, including crisis management, risk assessment in the telehealth environment, and experience in managing clients with chronic or severe mental health conditions
  • Possess an active RN Compact License
  • Possess excellent communication, organizational, and technological skills
  • Have experience with EMRs, Google Suite, Slack, Doxy.me, and RingCentral

Responsibilities

  • Evaluate potential crises identified by our Crisis Message Detection (CMD) system and follow the crisis response protocol when necessary
  • Manage a dynamic caseload of high acuity clients
  • Conduct ongoing assessments, care planning, skill-building, goal management, and psycho-education
  • Offer immediate crisis intervention for CCM clients in crisis situations; coordinate with emergency contacts and necessary resources
  • Take charge of researching, identifying, and negotiating community resources to help clients tackle social determinants of health
  • Spearhead care coordination efforts with a client's Cerebral care team, family, and other social supports, along with healthcare providers as needed
  • Facilitate ongoing clinical quality improvement projects

Preferred Qualifications

  • Hold a BSN and a Master's in social work, psychology, counseling, or a related field
  • Possess CCM certification
