Diabetes Clinical Advisor

Requirements

• Diabetes background as RN, RD/RDN, and/or CDCES with current licensure
• At least 3-5 years recent/current experience working with patients with diabetes
• Familiarity with DME related to diabetes, CGM, insulin pumps and direct work helping patients navigate these products
• Knowledge of current evidence-based diabetes care, including caring for diabetes-related lower extremity ulcers/wounds

Responsibilities

• Apply clinical expertise to contribute to Better Health’s peer support and care programs
• Collaborate with the Clinical Operations Manager to manage the quality of our peer support and care programs
• Assist in recruitment of peer coaches and care program staff
• Manage clinical quality for peer sessions through review of sessions and documentation and coaching of care program staff
• Conduct regular assessments of member outcomes and provide recommendations to drive improved outcomes for members
• Develop and maintain educational resources and tools for peer coaches and members

Preferred Qualifications

• Passionate about patient-centered care and a belief that peer support is an important component that supports and improves patient outcomes
• Experience with Google suite, knowledge management systems (e.g. Guru)
• Interested in flexible, contract-based work
