Summary

The job involves acting as a resource for Medicare patients throughout their healthcare journey, navigating them through complex health concerns, building trusting relationships, identifying and prioritizing patient needs, developing care plans, and learning new systems and software. The candidate must have an LCSW license in good standing, empathy, strong clinical skills, organizational skills, technical savvy, a bias towards action, and the ability to provide fearless feedback.

Responsibilities

• Act as a resource for Medicare patients via phone, email, and video throughout their healthcare journey
• Navigate patients through difficult and complex health concerns to help them achieve their health and wellness goals
• Be an empathetic listening ear and an action-oriented guide who knows what to do to solve patient problems—and actually does it
