Summary

The job is a full-time position for a Licensed Clinical Social Worker at CareBridge, providing telehealth services to Managed Long Term Support Services (MLTSS) patients. The LCSW will perform comprehensive psychosocial assessments and interventions, collaborate with the care management team, and provide targeted interventions for identified patients.

Requirements

  • Master’s degree in social work (MSW)
  • Licensed as an LCSW in the state of Texas with active NPI
  • Texas Medicaid and Medicare billing capabilities
  • 3+ years’ experience in medical social work; preference is given for behavioral health experience
  • Experience working with adult chronic disease patients and the geriatric population
  • Works and communicates well in a team environment
  • Maintains a patient-centered focus
  • Excellent communication skills
  • Basic computer and EHR skills and understanding

Responsibilities

  • Complete post-hospitalization assessments on select patients with mental health diagnoses within seven days of discharge and escalate to the psychiatrist or psych NP as appropriate
  • Respond to referrals from the CareBridge team promptly
  • Complete telephonic or telehealth psychosocial and economic assessment of patients
  • Support the needs of individuals, which can include counseling, crisis intervention, services, family therapy, and the mobilization of support networks
  • Assess for social determinants of health that may impact health
  • Collaborate with other CareBridge team members, health plan care coordinators, patients, and family to develop goals and interventions as appropriate
  • Deliver targeted interventions for identified patients
  • Provide patients with education on disease state, available services, and resources
  • Use motivational interviewing techniques to address non-adherence and quality of life concerns for patients
  • Support medication compliance and assistance with maintaining medical appointments
  • Facilitate patient access to community-based services
  • Coordinate services and referral to services when needed
  • Participate in interdisciplinary case conference discussions
  • Document assessments and plans in the CareBridge system

Preferred Qualifications

Medicare and Medicaid experience preferred
