Summary

Forge Health is a growing mental health and substance use provider seeking a passionate and dedicated mental health professional to join their team. The successful candidate will provide best-in-class mental health services via telehealth and in person, assessing clients for co-occurring needs, maintaining treatment plans, and communicating with families and outside resources. Requirements

• Valid, unrestricted license in New York as a LCSW or similar credentials including LMSW, LAC, LPC or LMFT
• Masters Degree in Social Work or a similar field

Responsibilities

• Facilitates traditional outpatient and intensive outpatient programs and services for Mental Health conditions, and substance use disorder
• Provides individual and group counseling
• Assesses clients for co-occurring mental health needs using DSM criteria
• Obtains and reviews records relevant to the current treatment episode
• Prepares and maintains treatment plans and necessary revisions
• Coordinates with discharge planner regarding continuing care and recommendations for discharge
• Documents all counseling services in clinical records
• Maintains all necessary logs required for administrative tracking
• Participates as a member of a multidisciplinary team assigned to the client and participates in case management meetings
