Summary

CareHarmony is seeking an experienced LPN Intake Coordinator for a fully remote role. The job involves conducting preliminary health assessments, identifying resources, coordinating needs, providing patient education, and assisting with medication management. Requirements include an active compact/multi-state license, technical aptitude, excellent communication skills, and a HIPAA-compliant workspace.

Requirements

• Active Compact/Multi-State license (LPN) (LVN)
• Technical aptitude – Microsoft Office Suite
• Excellent written and verbal communication skills

Responsibilities

• Accept transfers from the Patient Enrollment team to conduct preliminary health assessments for newly enrolled patients in our network
• Resolve patients' questions and create an open dialogue to understand needs
• Identify and coordinate community resources with patients that would benefit their care
• Provide patient education and health literacy on the management of chronic conditions
• Assist with medication management, including identifying potential medication concerns, adherence, and coordinating refills
• Assist in ensuring timely delivery of services to your patients; Home Health, DME, Home Infusion, and other critical needs

Preferred Qualifications

• Epic Experience
• Bilingual
• Additional Single State licensures
