Licensed Practical Nurse

Summary

Join CareHarmony as a Licensed Practical Nurse (LPN) and contribute to value-based care management. This fully remote, full-time position offers a consistent Monday-Friday schedule with minimal on-call responsibilities. You will manage patient census, coordinate community resources, provide patient education, and perform medication management. The ideal candidate possesses at least 3 years of direct patient-facing experience, a California LPN/LVN license, and technical aptitude. Requirements

• Active California license (LPN) (LVN)
• Technical aptitude – Microsoft Office Suite
• Must have active high-speed Wi-Fi
• Must have a home office or HIPAA-compliant workspace

Responsibilities

• Manage patient census with a resolution-driven approach to close gaps in clinical and non-clinical patient care
• Identify and coordinate community resources with patients that would benefit their care
• Provide patient education and health literacy on the management of chronic conditions
• Perform medication management, including identifying potential medication concerns, reconciliation, adherence, and coordinating refills
• Assist in ensuring timely delivery of services to your patients; Home Health, DME, Home Infusion, and other critical needs
• Resolve patients' questions and create an open dialogue to understand needs
• Assist/Manage referrals and appointment scheduling

• Epic Experience
• Bilingual
• Multistate/Compact License
• Additional Single State Licenses
