Summary

Join Mindoula Health as a licensed Therapist (LPC, LCSW, LMFT) in Tennessee to remotely serve our substance-exposed members across the U.S. Our Substance Use Disorder Program focuses on prenatal and postpartum care, supporting individuals facing substance-exposed living. You'll work in a family-focused culture, gaining clinical and business experience. Enjoy a flexible schedule, creating therapy groups and utilizing evidence-based practices. Requirements

Hold a LCSW, LMFT, or LPC license in Tennessee

Responsibilities

  • Assess, plan, and implement individualized care strategies directed toward the most appropriate and least restrictive level of care
  • Collaborate with the member, family, and healthcare providers to develop an individualized plan of care
  • Conduct individual counseling and group therapy with adolescents and adults
  • Identify and initiate referrals for social service programs – including financial, psycho-social, community, and state supportive services
  • Advocate for members and families to ensure the patient’s needs and choices are fully represented and supported by the healthcare team
  • Utilize approved clinical criteria to assess and determine the appropriate level of care for members
  • Document all member assessments, care plans, and referrals provided
  • Achieve set goals; Key Performance Indicators (KPIs)
  • Learn the StrongWell model and take responsibility and ownership for outcome-based care
  • Participate in interdisciplinary team meetings and utilization management rounds and provide information to assist with safe transitions of care
  • Promote responsible and ethical stewardship of company resources
  • Maintain excellent punctuality and attendance during work hours

Preferred Qualifications

  • Have experience with the substance abuse population
  • Have a background in maternal substance abuse
  • Have experience with adults and adolescents
  • Be familiar with Medicare and Medicaid procedures
  • Have remote work experience
