Requirements

• Graduate of an accredited Nurse Practitioner (NP) program (Master’s or Doctorate in Nursing)
• Board-certified as a Family Nurse Practitioner (FNP) or Adult-Gerontology Nurse Practitioner (AGNP)
• Active and unrestricted NP license in California
• DEA license or eligibility to obtain one
• CPR/BLS certification

Responsibilities

• Conduct telehealth assessments and consultations for senior patients
• Perform in-home visits to assess, diagnose, and treat patients in their living environments
• Manage acute and chronic conditions with a focus on geriatric primary care
• Develop and implement personalized treatment plans to improve patient health outcomes
• Provide patient and caregiver education on chronic disease management, medication adherence, and preventative care
• Coordinate with physicians, specialists, and other healthcare providers to ensure comprehensive care
• Document all patient interactions in an electronic health record (EHR) system
• Recommend and facilitate preventative screenings and wellness programs
• Participate in team meetings and case discussions to optimize patient care strategies

Preferred Qualifications

• Passion for geriatric care, chronic disease management, and telehealth services
• Comfortable working independently and traveling throughout Los Angeles County for patient visits
• Proficiency in telehealth platforms and electronic health records (EHR) (training provided if needed)
• Strong communication and interpersonal skills, with a commitment to providing compassionate patient care
