Remote Nurse Practitioner

Summary

The Nurse Practitioner at CareBridge provides primary and urgent health care to patients via telehealth modalities. They work closely with families, caregivers, and healthcare professionals to ensure patients receive necessary care. Responsibilities include managing patient care, ordering diagnostics, communicating effectively, documenting encounters, and participating in case conferences.

Requirements

• Holds active, unencumbered Advanced Practice license
• Must have an active compact RN license
• Credentialed in adult, family, or geriatric care
• Has active DEA license

Responsibilities

• Provide compassionate care to diverse patients and their families
• Manage the care of individuals with a multitude of health problems
• Thrive as a member of the interdisciplinary team
• Deliver cost-effective, high-quality care to patients
• Perform follow up and check in with patients
• Addresses Advance Care Planning proactively
• Manages patient’s medical care including ordering and monitoring diagnostics, referring to specialists, addressing recommended preventive measures, identifying acute changes of condition, managing medication therapy, communicating effectively with the care team, providing effective patient education, documenting all patient encounters, and participating in clinical case conferences

Preferred Qualifications

• Preference for: Developmental Disabilities Nurse certification
• Experience in care of adult, chronically ill patients
• Two or more years of practice experience of adult and chronic conditions
• Working knowledge of computers and ability to document effectively and efficiently in an electronic system
