Summary
Join PurposeMed's Freddie In-Person Care team as a compassionate Nurse Practitioner (NP) to provide comprehensive HIV treatment and prevention services. You will lead initial assessments, provide diagnostic services, manage treatment plans, deliver high-quality care, initiate and manage PrEP therapy, and counsel patients on sexual health. This hybrid role in Toronto requires strong clinical skills, excellent communication, and a commitment to reducing health disparities. As an independent contractor, you'll provide your own equipment and won't receive benefits offered to permanent employees, but you'll enjoy flexible hours. The role involves both in-person and virtual work, providing comprehensive care to patients living with HIV and those seeking preventative care.
Requirements
- A Master's degree in Nursing and a Primary Health Care or Adult/Acute Care Nurse Practitioner diploma
- Active Nurse Practitioner registration with the College of Registered Nurses of Ontario
- Minimum 1 year of practice experience as a Nurse Practitioner within the last 5 years, or 2+ years of Registered Nurse experience in HIV care, sexual health, or gender-affirming care
- Strong communication skills, empathetic care delivery, and a non-judgmental approach
Responsibilities
- Leading comprehensive initial assessments (for those living with HIV, or seeking preventative care), including but not limited to, history taking, risk assessment and psychosocial evaluations
- Provides comprehensive diagnostic services within preventative and HIV+ care, encompassing ordering of laboratory tests and diagnostics, phlebotomy and specimen collection, result interpretation, and clinical follow-up
- Managing treatment plans, using your advanced clinical skills to guide individuals living with HIV toward optimal health
- Deliver high-quality, evidence-based care—from initiating and monitoring antiretroviral therapy to managing co-morbidities
- Initiating and managing clients on PrEP therapy, including medication education, adherence counseling, and follow-up care
- Counselling and education on sexual health screening, treatment and prevention strategies such as DoxyPEP
- Championing health education and empowerment, helping patients understand their treatment options, navigate stigma, and take control of their wellness
Preferred Qualifications
- Experience working with gender and sexually diverse communities
- Experience providing HIV care
- Experience providing virtual care
- Experience working in the private sector, a start-up, or a technology company
- Experience working in an independent or autonomous practice setting
