Summary
Join Vida, a virtual health clinic, as a Prescribing Nurse Practitioner and provide accessible, convenient care to members via telehealth. You will partner with physicians to diagnose, manage, and provide medication management and outpatient treatment for various conditions. This role involves direct patient care, medication prescription, test ordering and interpretation, patient education, and communication. In addition to standard Monday-Friday hours, evening, morning, and weekend critical response outreach shifts are required. Requirements
- NP with active, unrestricted license in 15+ states
- Availability to cover at least three critical outreach shifts/week. Critical outreach shifts take place during the week early mornings(7am) or in the evenings(7pm) and also on the weekends. These shifts could last one hour or less
- Demonstrated experience working with patients, clients, and other clinical providers in an integrated setting
- Meticulous organizational skills and attention to detail
- Experience with digital technology and general computer skills
- Ability to perform independently, under minimal supervision with accountability for specific goals/objectives
- Demonstrates problem solving, leadership, conflict management and team building skills in order to ensure a productive work environment and achievement of goals
- Demonstrates strong aptitude to work in a rapidly changing, start-up environment
- Proficiency with EMR (electronic medical record) systems
Responsibilities
- Provide direct patient care and support by prescribing, creating, monitoring and supporting treatment plans in accordance with statutes, regulations and protocols
- Issue medication prescriptions in accordance with treatment guidelines
- Order laboratory tests, interpret and explain the test results to patients
- Provide patient education regarding medications, risks, benefits and reasonable outcome expectations
- Communicate with patients and engage in follow-up as necessary
- Refer higher acuity patients to specialized care based off of a stringent screening guideline to ensure quality of care and the best outcome for patients
- Maintain medical records and high quality of clinical documentation
- Provide medical care, patient education and medication prescribing and management in an empathetic and judgment-free manner
- Reach out to members identified by remote monitoring protocols and, if needed, prescribe rescue medications or direct to community medicine/emergency services
Preferred Qualifications
- Experience treating CHF, COPD, Diabetes and Obesity
- Previous experience with quality assurance and improvement practices
Benefits
$70 - $75 an hour
