Join Thyme Care as an Oncology Navigation Nurse Practitioner and become a critical member of our clinical team, providing virtual care to cancer patients. You will evaluate and manage members with urgent symptoms, develop comprehensive care plans, and collaborate with other healthcare professionals. Responsibilities include conducting annual clinical intakes, developing care plans addressing clinical and non-clinical needs, and overseeing interdisciplinary case conferences. This remote or hybrid role requires at least 3 years of NP experience, including 2 years in adult oncology with urgent care and telehealth experience. Requirements
- Have at least 3 years of NP experience, including 2 years in adult oncology with urgent care responsibilities and at least one year of telehealth experience
- Have an unrestricted license and a willingness to obtain additional state licenses as needed
- Be skilled in juggling multiple tasks and working under pressure without sacrificing organization in your communications and documentation
- Be winsome and articulate, but always start with listening and hearing what may not be voiced because you listen intently to others
- Build rapport and excellent working relationships with members and colleagues
- Understand that rapid changes to the business, strategy, organization, and priorities are par for the course in a start-up environment
- Have experience with video chatting, Google Suite, Slack, electronic health records or comfort in using and learning new technology
- Know how to identify and prioritize a member's needs, and do what it takes to immediately address urgent and important needs
Responsibilities
- Using a telemedicine platform, evaluate and manage members with urgent symptoms, complications related to their treatment, or exacerbations of their medical conditions
- Assess presenting symptoms, develop comprehensive care plans, triage to the appropriate level of care, educate patients, and ensure follow-up with relevant primary treating clinicians, including oncology and primary care
- Complete annual clinical intake visits for select members
- Develop care plans for members, in collaboration with onboarding RNs, to address clinical and non-clinical needs and barriers
- Prioritize membersโ goals and develop concrete plans for Oncology Nurse Navigators and Community Health Workers to reach these goals
- Review available medical records, work with external providers, give clinical insights to the care team, and adhere to best documentation practices to drive high-quality care for members
- Be responsible for the oversight and direction of interdisciplinary case conferences for members, in which individualized care plans are created and monitored by the team of Community Health Workers and Oncology Nurse Navigators
- Provide clinical oversight to the case management team and direct clinical care, as needed
- Participate in some holiday, night and weekend emergency coverage as needed
