Join Sollis Health as an Advanced Care Provider (Physician Assistant or Nurse Practitioner) and deliver exceptional virtual care to members in a concierge setting. You will work rotating shifts on the Aftercare and Triage teams, handling post-visit follow-up, telehealth consults, and responding to member inquiries via various channels. The Aftercare team focuses on post-visit medical care management, while the Triage team handles telehealth consults and clinical inquiries. This part-time, 100% remote opportunity requires a license to practice in FL, NY, CA, or WA (Sollis will assist with licensing in other states). Emergency medicine experience is a must. The role involves triaging patient needs, performing telehealth consults, providing in-person care when needed, and coordinating care with other healthcare providers. You will also provide exceptional customer service and maintain detailed documentation.
Requirements
- Bachelorโs Degree, required
- Graduate of an accredited PA or NP program, required
- If NP, RN degree required
- 3+ years of Physician Assistant or Nurse Practitioner experience in emergency medicine
- DEA license required
- NCCPA or Family Nurse Practitioner certification required
- Have outstanding customer service skills and a high level of professionalism
- Strong clinical skills
- Tech-forward and process oriented, with a goal of making patient care more efficient
- Ability to be adaptable and work in a fluid, multitasking environment
- Strong process and organizational skills
- Superb technical skills, including phlebotomy, peripheral IV placement, medication administration
- The ability to problem-solve and think outside of the box
- Be comfortable interfacing with five-star clientele
Responsibilities
- Triage incoming patients needs, assess their concerns and determine the best course of action including where their care should best be delivered - a Sollis center, virtually or via a requested house call visit
- Review medical inquiries from Sollis patients that arrive via email, text or phone, monitor navigator tasks and respond in a timely manner. Ensure appropriate documentation in our EHR
- Perform a telehealth/video consult or in-person evaluation by evaluating symptoms, prescribing medications, order labs and imaging as needed
- Provide care for patients in clinic, when requested
- Follow up with patients after they have been seen by a Sollis Health provider to assess their condition and determine if they need any additional care
- Follow up on send-out labs, imaging reports, monitor portals and navigator tasks
- Document clinical evaluation and treatment plans in Sollisโ EHR
- Assist patients with hospital admissions and transfers, as needed
- Interface with the patient's PCP or Specialist, or SH Medical Partners to discuss your assessment and plan and to coordinate follow up care after a Sollis encounter
- Work closely with your leader, Regional Medical Directors and other clinical team members to provide a comprehensive treatment plan and quality care
- Provide concierge level of care and customer service, aligned with Sollis Health Mission and Values
- Performs related duties as requested
