Nurse Practitioner, Women's Health (Certified Case Manager)

Join our team as a Women’s Health Nurse Practitioner and be part of delivering integrated virtual care and navigation, guiding members through complex medical situations, and ensuring they receive comprehensive care that results in positive health outcomes.

Requirements

• Active FNP or WHNP medical license and Board Certification
• At least 3-5 years of post-graduate experience in maternity care
• Be comfortable guiding maternity based care for a variety of members
• Obstetrics background and experience with management of infertility and menopause is highly valued
• Reside in RN compact state
• Experience with telehealth is highly valued

Responsibilities

• Deliver coordinated and compassionate virtual Care Management by telephone and/or video that improves members’ health outcomes
• Generate impactful care plans together with members, nurse care managers and our multidisciplinary care team, to help members achieve the desired goals
• Partner with the members’ local providers to ensure coordinated care. Coordinate necessary resources that holistically address members’ problems, whether clinical or social
• Provide clinical depth to a member’s care and help to identify opportunities to improve a member’s health and health outcomes
• Provide thought leadership for maternity care management program
