Obesity Medicine Nurse Practitioner

Join Form Health, a virtual obesity medicine clinic, as an experienced Nurse Practitioner to provide telehealth care for patients. The ideal candidate will have a Nurse Practitioner degree, 2+ years of experience in obesity medicine, and be licensed to practice in California. Requirements

• Have a Nurse Practitioner degree, with experience practicing Obesity Medicine, including effective evidence-based lifestyle counseling and pharmacotherapy
• Be licensed to practice and prescribe legally, and credentialed with insurance payers, in the state of California
• Have 2+ years of experience in outpatient or obesity medicine, endocrinology, or bariatrics, and/or 2 + years of obesity medicine experience in a primary care, internal medicine, or similar setting
• Have excellent interpersonal skills for patient and team interactions
• Be committed to the highest quality of medical care and an evidence-based approach to obesity management
• Have prior experience conducting telemedicine visits and who have a great "web-side" manner
