Requirements
- Be a Registered Nurse with a current unrestricted multi-state (compact) license (residence in a nursing compact state required)
- Be fluent in Spanish
- Have 4+ years experience in a hospital and/or healthcare practice, serving maternal-child health populations (minimum 3 years in labor and delivery)
- Be a flexible and agile thinker who embraces change
- Be internet-connected and able to work remotely via video, phone and text
- Be willing to travel occasionally (infrequent)
- Be willing to work nights, weekends and holidays
- Understand the prevalence of birth inequity and role that structural racism plays in maternal morbidity and mortality
- Be professionally engaged
- Be able to commit to the working hours: Three 12-hour overnight shifts per week (9:00 PM to 9:00 AM). Over a six-week period, you would work six weekend shifts total, which averages to about one weekend shift per week, though the exact schedule may vary. Additionally, there is a rotating holiday commitment, alternating between summer and winter holidays
Responsibilities
- Provide ongoing clinical and psychosocial assessment of new patients, providing reassurance and building rapport
- Identify and address barriers to care that have been identified by patients and/or the care team
- Reduce care gaps (missed appointments, medication management, etc.) by frequent and personalized engagements with patient
- Assess the need for and educate patients on the equipment they will use to monitor their health remotely
- Respond timely to abnormal diagnostic results (labs, radiology, etc.)
- Assess urgent concerns and proactively triage patients to support appropriate utilization of emergency services
- Document all care/interactions timely and escalate to appropriate multidisciplinary teams, as needed
- Support the development of programs and product by providing user feedback
Preferred Qualifications
- Have experience working with perinatal patients who have had complicated pregnancy-related conditions such as diabetes, hypertension, perinatal loss, etc
- Have experience providing virtual care
