Join Included Health as a compassionate and dedicated Physician generalist with Palliative Care fellowship training to support members throughout their healthcare journey. As part of a multidisciplinary team, you will provide holistic, patient-centered care through remote consultations and case management, developing personalized care plans and collaborating with local treating teams. Requirements
- Active medical license and Board Certification in Family Medicine (ABMS or AOA) or Internal Medicine (ABIM), with additional certification or experience in Palliative Care
- 3-year Family Medicine or Internal Medicine residency or equivalent
- 2 years of post-residency experience providing direct care to patients
- Willingness to become and maintain licensure in multiple states to meet program needs
- IMLC state residency
- Multiple state licenses
- Prompt and regular attendance at assigned work location
- Capability to remain seated in a stationary position for prolonged periods
- Eye-hand coordination and manual dexterity to operate keyboard, computer and other office-related equipment
- Capability to work with leadership, employees, and members in an appropriate manner
Responsibilities
- Provide holistic, patient-centered care through remote consultations and case management
- Develop personalized care plans
- Offer expert clinical guidance and empower members to make informed decisions about their health
- Manage complex conditions like cancer throughout the care journey
- Collaborate with local treating teams and coordinate resources
- Ensure seamless care transitions to improve health outcomes
- Analyze clinical trends and member data to contribute to program improvement and new ideas
Preferred Qualifications
- 3+ years of post-residency experience
- Ability to treat all age populations
- Licensure in Indiana, Alabama, Georgia, Mississippi, Missouri, Oklahoma, South Carolina, and Tennessee
