Summary
Join Evergreen Nephrology as a Dietitian and play a pivotal role in managing the nutritional aspects of patients in our Care Management program. You will collaborate with a team of healthcare professionals to develop and implement individualized care plans, provide dietary counseling and education, and connect patients with community resources. This remote position supports patients across multiple time zones. A Bachelor's degree in nutrition and Registered Dietitian certification with experience in kidney care are required. Requirements
- Bachelorβs degree in nutrition, dietetics, or similar area required
- Registered Dietitian with at least 12 months of clinical State licensure or certification is required if licensure or certification is available in the state where the local market is located
- Experience in caring for kidney care patients required
- Experience with team-based care, excellent collaboration skills
- Excellent working knowledge of community resources
- Exceptional capacity to multitask
- Self-directed
- Comfort with ambiguity
- Strong communication and teaching skills
- Intermediate skills with MS Office Suite of products including Outlook and Teams
- You have the ability to work effectively in a primarily remote environment: At minimum, a download speed of 25 Mbps and an upload speed of 10 Mbps is required; wired to the house internet (Cable, Fiber, or DSL) and hardwired to the internet device is recommended
- Team Members must work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information
Responsibilities
- Collaborate with nephrologists and other healthcare professionals to develop and implement individualized care plans that address the unique dietary needs of patients with kidney disease while aligning seamlessly with overarching care
- Serve as a key member of the interdisciplinary care team to follow a team- identified panel of patients until their needs have stabilized by delivering care interventions to patients or their care givers via telephone or other
- Support high risk, high need patients and their families by providing comprehensive diet education and counseling based on individualized patient needs including nutrient requirements, knowledge abilities, chronic medical, economic, and social conditions
- Provide specialized dietary counseling and education to patients and caregivers with CKD and ESRD, while focusing on managing nutrition intake to slow disease progression and improve outcomes
- Incorporate preventative care and early interventions while supporting interdisciplinary care team in executing customized care plans to reduce hospitalizations and improve long-term outcomes
- Develop and deliver educational materials and methods appropriate for patients to meet their unique dietary needs
- Check, track, and trend nutritional status, adherence to diet, laboratory values, and response to nutrition-related medications for appropriate interventions and status, adherence to diet, laboratory values, and response to nutrition-related medications for appropriate interventions and recommendations
- Connect patients to appropriate community agencies and resources, as appropriate
