Registered Nurse or Nurse Practitioner, Remote - Worldwide

Summary

The job is for a Registered Nurse or Nurse Practitioner with telehealth experience, licensed in multiple states, to oversee initial face-to-face visits and collaborate with the Clinical team at Carda Health. The role involves providing clinical care, health evaluations, diagnoses, lifestyle recommendations, note inputting, communication with other care team members, and process adherence.

Requirements

• Registered Nurse
• A Board Certified Nurse Practitioner
• Licensed in five or more of the following states (ideally all 50, but the more the better): New York, Florida, Pennsylvania, Virginia, Texas, Maryland, Illinois, Ohio, Michigan, Washington, Massachusetts, North Carolina
• Expertise in the management of individuals with cardiac & pulmonary pathophysiology
• Cardiopulmonary training in basic life support or advanced cardiac life support
• Experience prescribing and reviewing medication for cardiac and pulmonary patients
• Passion for delivering high quality care to patients
• Experience with care in a virtual setting and a desire to learn and influence new technologies
• Highly organized with the ability to create, adhere to, and scale repeatable processes

Responsibilities

• Provide high-quality clinical care utilizing best practices set forth by Carda’s Clinical team
• Be the first face-to-face touch point for many of the patients joining our program
• Perform health histories, evaluate patient records, diagnose, and refer to our programs when appropriate
• Provide lifestyle recommendations to patients related to specific goals
• Input notes and all other relevant information into our EMR
• Communicate and partner with other members of the patients’ care team to properly coordinate care and oversee patient care plans on an ongoing basis
