Requirements

  • Minimum of 3 years of nursing experience
  • Experience in Emergency and Triage trained
  • Telehealth or Critical Care (ICU) is highly desirable
  • Proven problem-solving skills with exceptional attention to detail
  • Strong organisational and time management skills, capable of working independently and within a team
  • Excellent communication and interpersonal skills, adept in both written and verbal exchanges
  • High level of self-motivation and proactive in enhancing personal performance
  • Confidence in using technology platforms and a commitment to ongoing professional development
  • Current registration with AHPRA without restrictions

Responsibilities

  • Conduct telephone and chat-based screenings to assess medical and mental health concerns
  • Determine the urgency and criticality of member conditions to facilitate appropriate referrals
  • Coordinate with mental health services, emergency services, and other healthcare facilities as needed
  • Make necessary appointments on behalf of members and ensure continuous care by working with multidisciplinary teams
  • Provide referrals to counselling and partner services according to member needs
  • Carry out follow-up calls to evaluate the need for further assistance and support continuity of care
