  • Clinical license must be active and in good standing
  • A minimum of three (3) years of professional experience dealing with patients with chronic illnesses
  • Master’s Degree as a Medical Social Work (MSW) required

Responsibilities

  • Plan and conduct continuing education programs
  • Conduct telephonic assessment of the patient's psychosocial status and assist in accessing resources for social, financial, and community needs
  • Identify and secure resources to meet the needs of the patient including financial, psychosocial, physical, and clinical resources
  • Provide information about and/or refer to other community resources that are available
  • Collaborate with CareHarmony team during urgent patient behavioral health concerns
  • Participate and support departmental quality improvement activities and projects
  • Act as a Social and behavioral health resource for the clinical team
  • Collaborate with providers and vendors to ensure patient is compliant with treatment plan
  • Develop resources, training and materials for staff and patients
  • Assist in developing protocols and defining best practice
  • Conduct training sessions with CareHarmony staff
  • Foster relationships within the team to ensure the CareHarmony culture is positive and supportive

Preferred Qualifications

  • Experience in clinical training and public speaking preferred
  • Experience building social work program preferred
