Join Belle, a rapidly growing engagement company revolutionizing home healthcare, as a remote Physician Assistant, Clinical Nurse Specialist, or Nurse Practitioner licensed in Georgia. The primary role involves conducting telehealth Annual Wellness Assessments and establishing Chronic Care Management (CCM) care plans for qualifying, polychronic members. This 100% remote position offers a flexible work environment, allowing you to leverage your healthcare expertise while maintaining a work-life balance. You will perform wellness evaluations, gather comprehensive patient histories, and collaborate with a dedicated team to improve patient health. The anticipated schedule is 9 am-6 pm EST, Monday-Friday, with no major holidays, on-call, or weekend work required. Belle offers competitive compensation and a supportive work environment.

Requirements

• Appropriate CNS, NP, PA licensure/certification in Georgia
• Willingness to obtain licensure/certification in additional states
• Compliant with requirements for CEUs and licensure in all states where a license is held
• Experience in telehealth, wellness visits, and preventative care is required
• Excellent communication and interpersonal skills, especially in a telehealth setting
• Strong organizational skills and attention to detail for effective patient documentation
• Comfortable utilizing and managing electronic health record (EHR) systems, medical software/technologies to ensure accurate documentation, and data security

Responsibilities

• Conduct telephonic wellness assessment visits
• Identify patient needs, provide health education, and offer lifestyle recommendations to support preventive health measures
• Create patient care plans
• Efficiently document patient interactions and maintain accurate records in compliance with company policies and healthcare regulations
• Collaborate with the care team to ensure patients receive appropriate referrals and care
• Identify and enroll patients who would benefit from CCM
• Review ongoing CCM patient care plans
