Requirements
- Certified and licensed as a Nurse Practitioner in Illinois
- Medicare participation and ability to have company bill for services on your behalf
- Certification from ANCC (or equivalent) as an Adult, Family, Geriatric and/or Acute Nurse Practitioner
- 1+ years of Nurse Practitioner Experience working with complex adult populations
- 1+ years of telehealth experience
- Ability to practice independently with little or no supervision
- Comfort using technology such as Google Suite, multiple EMRs, Slack
- Comfortable in a dynamic and always evolving start up environment
- English required
Responsibilities
- Perform comprehensive assessments of patients using telehealth platforms to gather medical histories and evaluate current health conditions
- Create individualized care plans that specify health monitoring parameters and outline achievable health goals for each patient
- Maintain accurate and timely documentation of patient interactions, assessments, education, and care plans within the EHR system. Ensure all records are compliant with policies and regulatory standards
- Assist in training other providers on remote monitoring workflow and best practices
- Collaborate with Skilled Nursing physicians, Circadia physicians, nurses, and other healthcare professionals to provide a comprehensive approach to patient care
- Participate in multidisciplinary team meetings to discuss and coordinate patient care strategies
