Join Pair Team as a Bilingual Community Health Worker (CHW) and make a real difference in the lives of underserved Medicaid recipients. Requirements
- Active CHW I Certification in the state of Nevada (required)
- Based in Clark County (Las Vegas) (required)
- Bilingual in English and Spanish (required)
- 1+ year of experience as a community health worker or in a similar outreach or care coordination role
- Reliable access to a vehicle
- Reliable internet connection and access to a quiet, HIPAA-compliant workspace
- Strong communication skills and cultural competency when working with vulnerable populations
- Ability to work independently in a remote environment while staying aligned with a cross-functional care team
- Organized, detail-oriented, and comfortable using digital tools (e.g., EHRs, CRMs, video conferencing platforms)
- Valid driverβs license and active auto insurance
Responsibilities
- Serve as a primary point of contact for a panel of members, maintaining consistent phone, text, and in-person outreach
- Build long-term, trust-based relationships to support engagement and continuity of care
- Help members navigate healthcare and social service systems, including housing, food access, transportation, and behavioral health
- Coordinate care activities, such as scheduling appointments, managing referrals, and ensuring timely follow-ups
- Collaborate closely with nurses, behavioral health care managers, and other care team members to ensure high-quality, integrated support
- Surface and help resolve barriers that may prevent members from accessing or continuing care
- Accurately document all member interactions and coordination activities in internal systems
- Meet outreach, documentation, and engagement targets in line with program requirements
- Participate in field-based member visits as needed to support engagement and outreach efforts
