Summary

Join Vesta Healthcare as a patient-focused Medical Assistant, coordinating care for members in Medicare's chronic care management program. You will make telephonic outreach calls, document interactions, and collaborate with the clinical team. Responsibilities include coordinating care for 20-30 members daily, managing referrals and prescriptions, and ensuring data accuracy. Requirements

  • Medical Assistant Registration or Certification
  • Fluency in English and Mandarin or Cantonese (writing, reading and speaking)
  • A minimum of 2 years of experience working in a healthcare setting, including a PCP, specialty clinic, or telehealth
  • The ability to work Monday - Friday, 8am-5pm ET, 9am-6pm ET, or 10am-7pm ET (some shifts/teams would be required one weekend per month)
  • Knowledge of referral processes, insurance coordination, and care coordination
  • Knowledge and understanding of chronic care management (CCM) processes
  • A customer service mindset for both internal and external customers
  • A strong proficiency in computer software navigation; data entry and data cleansing
  • A fundamental knowledge of Google Docs, Sheets, Slides or similar
  • A demonstrated ability to work effectively as a member of an interdisciplinary team, displaying good judgment and decision-making skills
  • Ability to perform duties as assigned or requested

Responsibilities

  • Provide practice support including: contacting members, caregivers, and care team members as directed, work closely with the clinical team to improve the health and care of our members
  • Coordinating care for 20 to 30 members daily, including referrals and prescriptions, ordering DMEs and supplies, coordinating transportation to and from medical appointments, and researching local resources for additional needs
  • Data entry within operating dashboards, reporting and workflow platforms
  • Data Quality and Accuracy via call and documentation quality audits
  • Other administrative support
