Care Navigator

Summary

The job is for a Care Navigator at Sana, a modern health plan solution. The role involves providing comprehensive care navigation support to members, collaborating with cross-functional teams, educating members on their care options, partnering with providers, conducting proactive outreach to high-risk members, and monitoring the effectiveness of care navigation strategies.

Requirements

• 2+ years work experience preferred, preferably in a fast-growth company environment
• 1+ years working in healthcare in some capacity (i.e., social work, managed care, insurance, etc.)

Responsibilities

• Deliver comprehensive care navigation and access support for Sana members
• Collaborate with cross-functional teams, including virtual care practice and customer support
• Educate members on their care referral options
• Partner with providers to secure member access to care
• Identify opportunities for provider contracting to expand network development and enhance member access and care navigation
• Build professional relationships with providers and Sana’s care partners to streamline referral pathways for care navigation
• Conduct proactive outreach to high-risk, rising-risk, and gap-in-care members
• Serve as a health advocate and supporter for members
• Monitor and assess the effectiveness of care navigation and access strategies
• Provide feedback to the product team to improve care navigation experiences for members

Preferred Qualifications

Prior Care Navigation experience strongly preferred
