Clinician

Summary

Join our primary care team as a dedicated Clinician, integrating behavioral health services. Collaborate with external organizations and behavioral health hubs to improve patient care. The ideal candidate is a nurse or social worker nearing PMP certification, possessing strong project management and data analytics skills. You will lead meetings, develop treatment plans, coordinate care, and contribute to quality improvement. This role requires conducting patient assessments and providing ongoing support and education.

Requirements

• Bachelor’s or Master’s degree in Nursing or Social Work
• Active clinical licensure (RN or LCSW)
• Almost completed Project Management Professional (PMP) certification
• Experience in data analytics and project management
• Strong communication and leadership skills

Responsibilities

• Conduct comprehensive behavioral health assessments and facilitate care integration
• Collaborate with third-party organizations to streamline services
• Lead and co-facilitate meetings, maintaining a focus on quality agendas
• Develop and implement patient treatment plans tailored to individual needs
• Contribute to quality improvement projects and performance metrics
• Provide ongoing support and education to patients and families regarding behavioral health issues
