Requirements
- Be Board Certified as a Family Nurse Practitioner in California
- Have an active Family Nurse Practitioner License in California
- Consistently exhibit the highest levels of professionalism, integrity, accountability, confidentiality, care and compassion to provide high quality health services
- Be willing to work in a revolutionary environment that sometimes necessitates last minute problem solving and out of the box thinking
- Be technologically savvy and comfortable using tools such as laptops or mobile devices for charting and HIPAA secure messaging apps for care coordination
- Possess strong written and verbal communication skills
- Be able to work independently or in a team environment
- Pass a national background check and a valid clinical license search
Responsibilities
- Review your case load and prepare your technology, ensuring you have all of the right tools available to service your patients
- Use easy and modern technology to navigate your schedule and partner with clinical in-home team members (Sprinters) who will visit each patientβs home
- Make a warm and welcoming connection with patients, collect relevant information, and perform various services
- Conduct thorough health risk assessments, medication reviews, cognitive screenings, and educate patients about their health and well-being
- Work directly with Sprinters to evaluate vital signs, arrange blood draws, and carry out necessary tasks to address the specific needs of the patient
- Provide comprehensive care coordination and management, including preventive care interventions, medication management, referrals to specialists, community resources, and documentation of findings
