Summary
Join Nest Health as a Licensed Clinical Social Worker and provide behavioral health services to families using telehealth. You will conduct initial evaluations, individual and family therapy, and collaborate with an interdisciplinary team. This remote role requires a Master's degree in a relevant field, an LCSW license, and at least two years of clinical experience. Requirements
- Possess a Master's degree in psychology or social science
- Hold a license in Clinical Social Work (LCSW) to practice in your state
- Have at least 2 years of clinical experience
- Have experience working with children, adults, and families
Responsibilities
- Conduct initial mental health evaluations resulting in realistic individualized treatment plans
- Complete assessments using standardized assessment/screening tools and measures
- Provide individual and family psychotherapy with children, adults, and families through virtual care modalities
- Provide brief, solution-focused therapy in compliance with internal policies, external governing statuses and guidelines including the Department of Children and Family Services and other pertinent institutions
- Conduct crisis assessments/triage with appropriate interventions
- Provide health education to members, care team, and practitioners within area of specialty, and other services related to wellness
- Review caseloads regularly to ensure appropriate utilization of services
- Attend team huddles and case conferences to advise on behavioral health needs and establish health goals as part of member's NAP (Nest Action Plan)
- Complete written documentation for all services rendered to members in Nest Health's electronic medical record (EMR) ensuring records are accurate, complete, client specific, and completed within specified timeframes
Preferred Qualifications
- Have experience providing mental health services remotely
- Have experience serving underserved populations with complex social needs
- Have experience working as a member of an interdisciplinary care team
- Be bilingual English/Spanish
- Have experience in a startup environment
