Summary

Requirements

  • Bachelor’s or Master’s Degree in social work
  • 3 years of clinical social work experience, part of a interdisciplinary care team
  • Valid and current LCSW license
  • Excellent listening and communication skills
  • Ability to make assessments and develop and implement care plans
  • Demonstrate a positive attitude and respectful, professional customer service
  • Acknowledge patient’s rights on confidentiality issues and follow HIPAA guidelines and regulations
  • Comfort in interacting as part of interdisciplinary team, confidence in speaking with physicians and medical staff regarding patient care needs
  • Comfortable with digital technology (including tools like Point of Care Tools, MS Office, Google, various EMRs, etc.) and able to troubleshoot technology issues
  • Ability to critically think, solve problems, and bring professionalism to all situations
  • Able to contribute to quality improvement and process improvement initiatives
  • Excellent written and verbal communication skills
  • Strong clinical and problem-solving skills
  • Ability to be flexible and embrace change while remaining highly organized, and detail oriented

Responsibilities

  • Serve on interdisciplinary care team as Behavioral and Social SME, as it relates to patients supported through Vytal Care programs; actively engage in patient rounds
  • Assesses patients using standardized assessment/screening tools, measures, etc
  • Provides solutions focused interventions
  • Documentation of services provided in accordance with requirements
  • Provides evidence based psychosocial interventions, including cognitive strategies, motivational interviewing, problem solving techniques, connection to community resources for resolution of identified SDoH needs
  • Provides psychoeducation
  • Providing remote telephonic services at a frequency based on risk and need
