Join our Clinical Team as a Licensed Practical Nurse (LPN) and become the central point of contact for patient care coordination. This full-time remote position, based in Georgia, requires at least two years of experience in clinical care coordination or healthcare management. You will manage communication between patients and healthcare providers, proactively reach out to patients for follow-up, and ensure accurate processing of care orders in the EMR. Collaboration with the care team to create personalized care plans and assisting the Practice Managers are key aspects of the role. Requirements
- At least two years of experience in clinical care coordination or healthcare management
- Current and valid Licensed Practical Nurse (LPN) certification in the State of Georgia
- Basic Life Support (BLS) certification
Responsibilities
- Be the communication hub for the entire care teamโpatients, caregivers, doctors, or external health professionals
- Advocate for patients by managing communication between them and healthcare providers, helping them navigate their care and ensuring they receive the support they need from home health, hospice, social services, and more
- Proactively reach out to patients with scheduled follow-up calls, health check-ins, and care coaching
- Take ownership of care orders, ensuring all tasks are accurately processed in the EMRโfrom scheduling appointments to managing prescriptions and coordinating home health services
- Collaborate with the care team to create personalized care plans for each patient
- Be a team player by stepping in to help the Senior and Associate Practice Managers as needed
- Get out into the field with the PCP to assist with basic care functions, and sometimes work independently to engage hard-to-reach patients for important HEDIS measures
Preferred Qualifications
- Fluency in both English and Spanish, with strong communication skills in speaking and writing
- Comfortable using technology, especially electronic medical records (EMRs)
- Experience working with Athena EMR
- Enjoy working with people from diverse social, ethnic, and cultural backgrounds and are passionate about providing care that meets their unique needs
