Summary
Join Mindoula Health as a licensed Therapist (LPC, LCSW, LMFT) in Tennessee to remotely serve members in our Substance Use Disorder Program focusing on prenatal and postpartum care and substance-exposed populations. You will work remotely, creating a flexible schedule and utilizing evidence-based practices. Collaborate with a team to provide assessments, individual and group therapy, and coordinate care. Enjoy the autonomy of working from home while contributing to a family-focused culture and learning both clinical and business aspects. Compensation includes the potential to earn up to $75,000 annually, plus a comprehensive benefits package.
Requirements
Be a licensed LCSW, LMFT, or LPC in Tennessee
Responsibilities
- Assess, plan, and implement individualized care strategies
- Collaborate with members, families, and healthcare providers to develop individualized care plans
- Conduct individual counseling and group therapy with adolescents and adults
- Identify and initiate referrals for social service programs
- Advocate for members and families to ensure their needs are met
- Utilize approved clinical criteria to assess and determine appropriate levels of care
- Document all member assessments, care plans, and referrals
- Achieve set goals and Key Performance Indicators (KPIs)
- Learn the StrongWell model and take responsibility for outcome-based care
- Participate in interdisciplinary team meetings and utilization management rounds
- Promote responsible and ethical stewardship of company resources
- Maintain excellent punctuality and attendance
Preferred Qualifications
- Have experience with substance abuse populations
- Have a background in maternal substance abuse
- Have experience with adults and adolescents
- Be familiar with Medicare and Medicaid procedures
- Have remote work experience
