Summary
Join Lumata Health as a myVisionTrack (mVT) Specialist and play a vital role in supporting patients enrolled in the mVT program. You will monitor patient app usage, follow up with patients to ensure adherence to testing recommendations, and maintain accurate documentation. This full-time remote position requires strong communication and organizational skills, along with experience in a patient-facing healthcare role. The ideal candidate will possess COA certification and ophthalmology experience. Requirements
- Minimum of 2 years in a patient-facing role within a healthcare setting, with a focus on patient education and support
- Strong organizational and documentation skills
- Proficiency in using mobile applications and digital health tools
- Excellent verbal and written communication skills
- Strong interpersonal skills with a patient-centric approach
- Understanding of regulatory requirements related to patient data and confidentiality
- Strong attention to detail when tracking information and updating documentation digitally
- Completion of required mVT training videos provided by Lumata
- Commitment to continuous learning and staying updated on new developments related to the mVT app and vision monitoring practices
- Must have a home office that is secure and free of distractions (HIPAA-compliant workspace)
- Prolonged periods of standing, sitting at the desk and working on a computer
- Must be able to hear, speak, call and virtually meet
Responsibilities
- Monitor patients' usage of the mVT app
- Download patient reports from the mVT portal
- Document app usage appropriately in Salesforce
- Follow up (via phone and/or text) with patients who are non-adherent with testing through the app
- Achieve 80%+ patient adherence to physician-recommended testing frequency
- Ensure seamless communication between patients, Care Coaches, and healthcare providers
- Follow established triage process for mVT alerts
- Complete required mVT training videos
- Stay up-to-date with app changes and updates
- Communicate changes to Care Coach team as necessary
- Collaborate with other team members, including mVT Onboarding Specialist(s) and Care Coaches, to ensure seamless patient communication and increase patient satisfaction with mVT app
- Collect mVT program feedback from patients and providers
Preferred Qualifications
- COA certification
- Experience in Ophthalmology
