Obesity Medicine Nurse Practitioner

Join Form Health, a virtual obesity medicine clinic, as an experienced Nurse Practitioner to provide telehealth care for patients with obesity. Collaborate with physicians and registered dietitians to deliver exceptional team-based care in this full-time, remote position with flexible scheduling. You will leverage technology to enhance the patient experience through video visits, text messaging, and other digital tools. The role requires experience in obesity medicine, including lifestyle counseling and pharmacotherapy, and a commitment to evidence-based care. Bilingual candidates with medical fluency in Spanish are highly preferred.

Requirements

• Have a Nurse Practitioner degree, with experience practicing Obesity Medicine, including effective evidence-based lifestyle counseling and pharmacotherapy
• Are licensed to practice and prescribe legally, and credentialed with insurance payers, in at least one of the following states: MA, NY, TX, NJ, and/or IL
• Have 2+ years of experience in outpatient or obesity medicine, endocrinology, or bariatrics, and/or 2 + years of obesity medicine experience in a primary care/internal medicine setting
• Have excellent interpersonal skills for patient and team interactions
• Are committed to the highest quality of medical care and an evidence-based approach to obesity management
• Have prior experience conducting telemedicine visits and who have a great "web-side" manner

Preferred Qualifications
