Requirements

  • Bachelor’s degree in Nursing
  • Active RN licensure in state hired
  • Strong experience with in depth knowledge of oncology disease processes and treatment plans
  • Reside in a Nurse Licensure Compact (NLC) state
  • At least 5 years of experience in clinical and managed care roles such as case management or care navigation
  • Exceptional verbal and written communication skills
  • Demonstrated customer service, communication, and organizational skills
  • A passion for helping people and finding unique ways to communicate care plans and ideas
  • Ability to work independently, prioritize effectively, and make sound decisions

Responsibilities

  • Research and analyze member health needs to develop an effective care plan for the members to reach goals and obtain optimal health outcomes
  • Leverage your clinical experience, perceptive business sense, and critical thinking skills to optimize resources and guide members to cost effective outcomes
  • Combine your experience of evidenced based practice with being innovative to drive new solutions for the best member experience
  • Educate members or member representatives to empower them in making better healthcare decisions for higher quality care while lowering costs for the member and plan
  • Collaborate and communicate with all members of the healthcare team from the member, to the family, and other team members at Gravie
  • Communicate with external partners including providers, vendors, and network representatives
  • Document and follow procedures according to the standardized processes in place
  • Demonstrate understanding of healthcare benefits and how they play a role in member behavior so you can navigate the healthcare system
  • Use your knowledge of case management, disease processes, chronic conditions, and complex situations to help identify the next best actions for members
  • Be flexible and excited about change in a fast-paced, growing environment
  • Demonstrate a strong sense of accountability for clinical program success
  • Create effective relationships, listen, communicate and be a helpful team member across the organization
