Requirements

• Valid license to practice as a Physical Therapist in the state
• Doctorate or Master's degree (MS) in Physical Therapy from an accredited institution
• Minimum of one (1) year of professional experience as a Physical Therapist
• Current CPR certification
• Possession of a valid driver's license and access to a reliable, insured vehicle
• Excellent communication skills and ability to work independently

Responsibilities

• Conduct comprehensive physical therapy assessments and evaluations in patients' homes
• Develop personalized treatment plans and goals based on individual patient needs
• Deliver direct physical therapy services and interventions to improve mobility, strength, and functional independence
• Collaborate with interdisciplinary teams to ensure coordinated and holistic patient care
• Educate patients, families, and caregivers on therapeutic exercises and techniques for ongoing management
• Maintain accurate and timely documentation of patient progress and treatment outcomes
