Requirements

Responsibilities

• Provide virtual education, care coordination, and follow-ups for cardiology patients
• Complete 2-day on-site training in Bethlehem, PA
• Work with EMR and telehealth tools to streamline patient tracking and reduce administrative tasks
• Provide direct patient care without management responsibilities
• Collaborate with physicians, APPs, and support staff to ensure optimal patient care
