Requirements

• Current and valid Registered Nurse (RN) license in the state of Nevada
• Minimum of 1 year of experience in home health or a related field
• Strong clinical and assessment skills
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a collaborative team

Responsibilities

• Conduct initial assessments and develop individualized care plans for patients
• Coordinate and provide skilled nursing care in a home setting
• Collaborate with interdisciplinary healthcare teams to ensure the best possible patient outcomes
• Educate patients and their families on healthcare management and promote self-care
• Monitor and evaluate patients' progress and adjust care plans as needed
