The job is for a part-time contract care manager who will provide digital cardiac rehab services in Tagalog/Pilipino. Requirements

• Minimum Bachelor degree in exercise related science
• Must speak Tagalog or Pilipino
• Certifications required: - Exercise related science: ACSM-CEP (preferred CCRP certification or Master's degree or willing to sit for the CEP and/or CCRP exam within 6 months of hire)
• 1-5 years of prior experience in cardiac education, care management, cardiac rehab and secondary prevention required

Responsibilities

• Guide participants in 1:1 video calls and group education sessions through our 3 month online program to help them manage their cardiac related risk factors
• Develop comprehensive treatment plans that are tailored to the individual risk factors and medical needs of the patients
• Engage patients weekly to provide coaching, education, emotional support and modify treatment plans through video, voice or text messages
• Coordinate the approval of treatment plans remotely with Medical Directors
• Provide progress reports on an as needed basis when symptoms are reported or vital signs are not at goal
• Share approved treatment plans to referring providers on a monthly basis to provide insight into the patient’s performance in the program
• Monitor dashboard for adverse events and triage appropriately
• Leverage our cutting-edge platform to strategically coach participants through the Movn program, using data and analytics to inform outreach methods during moments that matter
• Collaborate with our Clinical, Data, Product and Operations teams to discover and drive digital health coaching best practices
