Summary

Requirements
- Active Registered Nurse (RN) license in Arizona
- Possess a valid and current driver's license
- Strong background in geriatric/senior care, with a focus on care assessments and care coordination
- Experience in care planning for chronic conditions, including collaboration with interdisciplinary teams
- Excellent documentation and electronic medical record-keeping skills
- Ability to work autonomously, solve problems, and adapt to new processes as the role and organization evolve
Responsibilities
- Conduct comprehensive assessments of residents within senior living facilities, identifying care needs and gaps
- Assist in developing and implementing chronic condition care plans in collaboration with the clinical team
- Coordinate care across multiple stakeholders, including the facilityβs on-site teams and Savoy Life's medical group
- Collaborate with the Director of Nursing, Assistant Director of Nursing, and Health and Wellness Directors to ensure seamless care delivery
- Maintain accurate and detailed documentation in both the facilityβs Electronic Health Record (EHR) system and Savoy Life's Electronic Medical Record (EMR)
- Provide clinical expertise and act as a resource for on-site staff to support high-quality resident care
- Operate with flexibility and adaptability in a new and evolving role within a growing organization
Preferred Qualifications
- Experience in working in senior living facilities
- Strong clinical assessment and care planning skills
- Ability to work closely with both clinical and facility teams to coordinate care
- Proficiency in using EHR and EMR systems for documentation and reporting
- Excellent communication skills and the ability to work collaboratively with a diverse team
- Adaptability in a new role and organization, with a focus on innovation and resident care
