Requirements

• Master’s or Doctoral degree in Nursing from an accredited institution
• Active Nurse Practitioner (NP) license in the US
• Geographically located in the US
• Strong clinical knowledge and experience in managing diabetes and/or obesity
• Commitment to maintaining high standards of professionalism, ethics, and patient safety in a virtual care setting
• Reliable internet connection and a dedicated, distraction-free workspace

Responsibilities

• Conduct a combination of synchronous and asynchronous consultations with patients diagnosed with cardiometabolic conditions
• Perform comprehensive medical assessments, review patient medical histories, and order appropriate diagnostic tests as needed
• Develop individualized treatment plans tailored to each patient's unique needs
• Prescribe medications, lifestyle modifications, and other therapeutic interventions to optimize patient health outcomes within the scope of nurse practitioner practice
• Educate patients about their conditions, treatment options, medication adherence, and preventive measures
• Monitor patient progress through regular follow-up appointments, remote monitoring devices, and virtual check-ins
• Collaborate with our team of dietitians, health coaches, pharmacists, physicians, and patient care advocates
• Utilize telehealth technology and a custom electronic health records (EHR) system efficiently to document patient encounters, update medical records, and communicate with the care team
