Summary

Join Accompany Health in making a tangible difference in people's lives and helping correct long-standing disparities in healthcare as the Behavioral Health Manager (BHM). This role will be an important leader in clinical operations, managing BHCs across multiple markets, and serving on the central clinical operations team. The BHM will work closely with the National Director of Integrated Behavioral Health to ensure that our BHCs are performing to the top of their license and working in a supportive environment.

Responsibilities

  • Recruitment of new BHCs, including interviewing and providing feedback to our talent team
  • Retention of our current BHCs through mentorship and career development
  • Coordination of onboarding of new BHCs in conjunction with our team of learning and development specialists
  • Developing and tracking performance reviews of BHCs, including quarterly bonus metrics
  • Identifying and implementing processes to reach quarterly KRs by partnering with the National Director of Integrated Behavioral Health
  • Providing feedback and behavioral health programs policies and processes to improve the quality of care
  • Provide feedback, input, and testing for future product builds
  • Assist in wellbeing and diversity, equity, and inclusion initiatives
  • Monitor quality and clinical documentation for the BHC team
  • Attending quarterly clinical off-site meetings
  • Assessing need for the behavioral health program including future service lines and initiatives, in coordination with our National Director of Integrated Behavioral Health
  • Co-develop behavioral health programming for training of BHCs and Accompany Health employees on pertinent behavioral health topics
  • Assist in developing market-based behavioral health referral resource guides
  • Working with the central clinical operations team in other measures not mentioned above
  • Quarterbacking our integrated behavioral health service which closely aligns with the measurement-based treatment to target Collaborative Care Model, for patients with mild/moderate behavioral health needs including depression, anxiety, PTSD, and alcohol/opioid substance use disorders
  • Patients in this model are directly referred by our primary care team or identified via data analytics in our patient population to meet the above criteria. Our BH clinicians engage patients in our intervention and track patients in a registry to monitor treatment progression
  • Duties of the BH clinician in this model include but are not limited to: outreaching and engaging referred/qualified patients, accepting (and at times initiating) warm handoffs from other team members, completing a clinical initial intake with in-depth psychosocial history and clinical diagnosis, formulating a comprehensive treatment plan in partnership with the psychiatrist, executing on the treatment plan in coordination with our primary care teams, delivering brief therapeutic interventions and short-term therapy, proactively monitor and track patients via a care registry to insure clinical improvement, and referring and navigating to community resources for access to the broader behavioral health continuum of care
  • Quarterbacking our Bridge Services model for patients with serious mental illness (e.g. schizophrenia, bipolar, etc.) and/or substance use disorder
  • Patients in our Bridge Services model receive short-term intervention (3-6 months) from our BH clinician and psychiatrist in order to stabilize and transition to long-term BH services in the community
  • Quarterbacking our Low Intensity Services model for patients with behavioral health concerns that are not severe enough for collaborative care or bridge services
  • Patients in our Low Intensity Services receive brief (1-3 session) intervention, with referral to the community of further care is needed. This also includes helping coordinating with our consulting Psychiatrist for virtual Psychiatric eConsults for strictly medication recommendations or chart review
  • Supporting patients in transitions of care from facilities for admissions related to behavioral health diagnoses, including but not limited to: coordinating on discharge plans with inpatient care management, performing assessments to identify risk factors for readmissions, ensuring close follow-up and coordinating services
  • Assisting the clinical team in urgent triage for behavioral health related issues
  • Participating in multi-disciplinary case conferences and clinical rounds to ensure holistic and appropriate care for patients
  • Providing formal and informal coaching and education to other clinical and non-clinical members of multidisciplinary teams on excellent patient care
  • Flexibility in day-to-day variations of processes that occur as a clinical consultant and people manager
