Requirements

  • Masters’ degree with major course work in social work
  • License/certification eligible or Licensed (LMSW) preferred in Texas/Florida
  • Minimum 3-5 years of post-graduate experience in health care social work/Case management in behavioral health Required
  • Experience working with pediatric population and family systems required
  • Proficiency in motivational interviewing practices and/or techniques; goal setting and intervention; assessment of needs
  • Knowledge of social work including crisis prevention and intervention
  • Experience with providing telehealth services
  • Knowledge of outlook, Microsoft office, and ability to work in online platforms

Responsibilities

  • Monitor high-risk pediatric patients, ensure appropriate follow-up and clinical management, adjust care plans as needed
  • Evaluate effectiveness of care plan and identify need for transitions of care to most appropriate care setting
  • Develop a plan for intervention that is consistent with the social/emotional/physical needs of patients and caregivers
  • Provide case management and support that requires clinical expertise in various systems with focus on helping patients and families negotiate the complexities involved with a mental health diagnosis
  • Support patient and family system in accessing appropriate, quality care
  • Provide interventions in response to crisis to de-escalate and stabilize patient and family members
  • Participate in ongoing scheduled consultations with an interdisciplinary team to monitor patient progress
  • Represent Imagine Pediatrics commendably to patients, families, providers, and community
  • Provide telehealth visits
  • Perform other duties and assume other responsibilities as assigned by manager
