Summary

Colla Health is a company focused on improving the quality of life for cancer patients through behavioral health care. The Behavioral Health Care Manager (BHCM) will deliver care to cancer patients using the Collaborative Care Model, develop relationships with partner oncology practices, and provide brief behavioral interventions tailored for the cancer setting. The BHCM must hold a specific license, have prior experience collaborating with a multidisciplinary team, and demonstrate effective communication skills.

Requirements

  • Hold one of the following licenses: Licensed Clinical Social Worker (LCSW), Licensed Master Social Worker (LMSW/LSW), Clinical Professional Counselor (CPC), Licensed Professional Counselor (LPC) Licensed Psychologist (PH.D or PsyD), or Certified Oncology Social Worker (OSW-C)
  • Have a passion for delivering quality behavioral health care

Responsibilities

  • Develop a trusted, effective working relationship with the partner oncology practice team
  • Support patients, clinicians, and practice staff with the identification and intake of patients needing behavioral health support
  • Provide brief behavioral interventions tailored for the cancer setting
  • Screen and assess patients for common mental health and substance abuse disorders
  • Facilitate patient engagement and follow-up care
  • Provide patient education about common mental health and substance abuse disorders and the available treatment options
  • Systematically track treatment response and monitor patients for changes in clinical symptoms and treatment side effects or complications
  • Support psychotropic medication management as prescribed by medical providers, focusing on treatment adherence monitoring, side effects, and effectiveness of treatment
