Requirements

  • Fluency in English and Spanish is required
  • Previous experience in a patient care coordination role or similar position in the healthcare industry
  • Strong knowledge of healthcare processes, referrals, and insurance verification
  • Excellent communication and interpersonal skills
  • Strong organizational skills and attention to detail
  • Ability to handle multiple tasks and prioritize effectively
  • Proficiency in using electronic medical record (EMR) systems and other office software
  • Ability to work independently and as part of a team
  • Experienced in providing customer service in a healthcare setting

Responsibilities

  • Coordinate the new patient referral process, including managing paperwork and documentation
  • Facilitate communication between new patients, referring providers, and specialist offices
  • Assist with scheduling patient appointments and follow-up visits
  • Provide information on services and resources available to new patients
  • Translate and interpret documentation to ensure clear understanding for patients and providers in both English and Spanish
  • Maintain accurate and organized records of referrals and patient interactions
  • Assist patients with inquiries and concerns related to the referral process
  • Collaborate with healthcare providers and administrative staff to ensure smooth patient transitions
