Care Coordinator

Summary

Join NuvoAir as a Care Coordinator and be the first point of contact for patients after CCM/PCM/RMP is ordered. You will proactively identify and work with individuals, particularly the elderly and those with long-term cardiopulmonary conditions, to coordinate and navigate their care. Responsibilities include developing personalized care plans, managing patient needs via phone, conducting and documenting visits using technology platforms and EHRs, and connecting patients with necessary resources. This role requires healthcare experience and a startup-ready mindset. Requirements

• Healthcare experience as a background in the medical field is a must have
• Exhibits a startup-ready mindset, adaptable to a dynamic environment where patient needs are prioritized

Responsibilities

• Accountable for helping patients keep up with their care plans
• Coach patients to reach their care goals by following treatment plans that include healthy eating, physical activity, stress reduction, etc
• Develop personalized care plans to address patients’ physical, mental, and preventative health needs
• Manage and support patients’ healthcare needs via phone conversations
• Conduct and document visits by using several technology platforms and EHRs
• Connect patients with resources such as nurses, respiratory therapists, and doctors who can also assist them in meeting their care needs

Preferred Qualifications

• Passionate about health and wellness, often engaging in conversations about healthy habits
• Comfortable in engaging conversations, and known for making others feel at ease
